KINDLY ENCLOSE THE FOLLOWING DOCUMENTS
1-Change-Correction Form
2- Attested copy of N.I.C
3- Attested copy of the payslip
4- Application (Hand written) to process change (Address to Managing Director PGSHF)

INSTRUCTIONS
1) Complete Procedure should be followed.
2) Please fill only that line of information which you want to change.
3) Signature on application or change form should match with N.I.C
4) Applications are acceptable only by postal mail or by hand.
5) No request will be entertained for change if sent electronacilly by e-mail, fax or through PGSHF web site.
